1. Patron Eligibility
    1. This service is available to those who live within four miles of the City of Wautoma who are temporarily
      or permanently unable to visit the library due to an illness, disability, or other qualifying
      circumstances.
    2. Patrons must fill out the Wautoma Public Library Home Service Application to be considered
      for the program.
    3. The patron must be in good standing to qualify for this service.
  2. Library Card Registration/Use
    1. Each home service patron must possess or register for a library card if s/he does not already have one.
    2. A file containing home service registration forms, including library card number, will be maintained at
      the circulation desk for use in checking out materials to these patrons.
    3. If the patron does not have a valid card, an application will be filled out by phone or in person and the
      librarian/volunteer will bring the card application and the home service application with them for the
      patron’s signature on the first delivery date.
  3. Delivery Schedule/Loan Period
    1. Materials will be delivered approximately once a month. The schedule of delivery is up to the discretion
      of the delivery volunteer to coordinate. At the time that new materials are delivered, the items from the
      previous delivery will be retrieved and returned to the library.
  4. Fines/ Fees
    1. There is no fee for home service. Overdue fines will not be charged on materials, but the library’s
      standard fee schedule will apply for damaged or lost items.
    2. The Wautoma Public Library retains the right to discontinue service if borrowed items are lost, damaged,
      and/or not returned in the same condition as they were delivered.
  5. Materials Available for Home Service
    1. All formats of materials are eligible for home service, excluding electronic devices.
    2. Each delivery will be limited to one bag of materials.
    3. If a material desired is not owned by the Wautoma Public Library it can be requested by contacting library
      staff at (920) 787-2988 or logging into your account online.
  6. Renewals
    1. If a material is desired longer than the four-week delivery schedule, then a patron should contact library
      staff at (920) 787-2988 or log into the library account online for a renewal.
  7. Environment Required for Delivery
    1. Volunteers are unable to assist patrons with other errands, household chores, etc. They are there for library
      purposes.
    2. Volunteers are not to search for missing or misplaced library items. It is the responsibility of the Patron to
      keep track of all materials for return.
    3. Volunteers or staff members will exchange library materials at the door.
    4. Patrons requesting Home Service must provide a safe and appropriate environment for all volunteers and staff
      members who make deliveries to their homes. Home Service Patrons will make sure that:

      1. Pets are confined (with the exception of service animals trained to assist a disabled person).
      2. The path to the home is clear and safe, with snow shoveled and ice removed.
      3. The person answering the door is dressed appropriately.
      4. The person answering the door refrains from abusive or obscene language, making obscene gestures or
        displaying obscene images.
      5. The person answering the door is not exhibiting signs of illness that may jeopardize the health of the
        library’s representative.
    5. If a volunteer or staff member must leave the home, deny service, or wishes to recommend suspension of service
      because the occurrence of any of the above is deemed to make the home environment for delivery unsafe or
      inappropriate, the volunteer or staff member shall provide the Wautoma Library Director with notice of why
      such action occurred together with any recommendation for length of suspension of service.
    6. The Wautoma Library Director shall send written notice to the patron of the reason for and the length of any
      continuing suspension of service.
    7. Any homebound patron may request in writing that the suspension of service be reviewed by the Board of Trustees at
      the next monthly Board Meeting.
